Patient referrals to Newsome Physical Therapy can be made by your physician, nurse practitioner, rehab nurse, case manager, employer, insurance carrier or by patient request. Once you have obtained a written referral for physical or occupational therapy we will offer you an appointment within 24 hours.

First Visit
-Prior to your first visit the billing staff will verify your insurance benefits with the information you provide during the initial intake.

-We ask that you arrive 15 minutes prior to your appointment time to complete the necessary paperwork, or you can print the necessary forms: medical history form and basic patient form, complete them prior to your first visit. We also ask that you bring the written order/prescription for physical therapy, your insurance card(s) and driver’s license.

- One of our highly trained physical or occupational therapists will then conduct an initial evaluation. The therapist and patient will set goals that will be attained during the course of treatment. The findings of this evaluation will be summarized and a report will be sent to your referring physician.
